By signing, each of the above listed business and business owner officers/officials (individually and/or collectively "you") authorize 360 Finances LLC and each of its representatives, associates, vendors, designees and business associates with whom 360 Finances LLC has, or may in the future enter into, business relationships ("Recipients") to: (1) obtain consumer, business, and investigative reports and other information about you, including credit card processor statements, and bank statements; (2) obtain credit card processor statements, and bank statements from bank servers and other third parties; (3) obtain tax returns by any means; (4) release information relating to your business; (5) transmit this application form, along with any of the foregoing information obtained in connection with this application, to any Recipient; and (6) contact you via email, call, and/or text message at the email address and/or phone number provided above.
